50%   - Complete Series or Panoramic      
100% - Periapical x-rays    
100% - Bitewings (1 time per year)
100% - Routine prophylaxis (cleaning), (2 per year)
100% - Periodontal Maintenance (2 per year)
20%   - Additional Cleanings or Periodontal Maintenance visits per year 
100% - Fluoride (2 per year, no age limit)
